1. The grout has turned a colour it wasn't born with
Grout that's black, pink, or mystery-grey isn't just ugly. It's usually harbouring mould. Surface cleaning won't fix it. At a certain point, replacement is faster and cheaper than trying to save it.
2. The caulking is separating
A gap between your tub and the wall is an open invitation for water to get behind the tile. Water behind tile is a bad day that gets worse slowly. Left long enough, it takes the subfloor with it.
3. Your vanity was clearly someone's idea of modern... in 1996
Aesthetics matter. You spend real time in there every morning. A dated bathroom affects how you feel about your house, and what it's worth when you sell.
